It appears wherever the code is pasted. If you put it in the general template it shows on the whole site; if you paste it on a single page, only there. It's the most common mistake: pasting it on one page and expecting the visitor to see it when they come in through another.

Yes, from your portal and with no paperwork. When you disconnect, the employee stops using that information right away and the rest of the conversation keeps working the same. Nothing stays tied down and there's no minimum term.
